Requesting High School Transcripts  

Authorization to Release Information - Google Form

You will need to upload a photo ID with this request. If you have  any questions regarding this release form or the requirement of a photo ID, reach out to Wendy Crunkleton at wendy.crunkleton@d11.org or call her at 719-328-2025. Please allow 5–7 business days for your request to be processed.

  • PLEASE DO NOT give the counselor’s email address-that will delay the process
  • PLEASE DO NOT contact the Counseling Office-that will delay the process
